Coach Application Process
Conducted at 7630 Hayward Road, Frederick
TEACHER (CERTIFIED) COACHES*
o Completed coach application (a triplicate form picked up at your local high school) – athletic director must sign
o Fingerprinting and tax forms already processed
EMERGENCY (PAID) & VOLUNTEER (UNPAID) COACHES*
o Completed coach application (a triplicate form picked up at your local high school) – athletic director must sign
o Two (2) forms of original ID (driver’s license, SSN card, birth certificate, passport) — does not apply to teachers already in system
o We strongly encourage tax forms completed in case status changes from volunteer (unpaid) to emergency (paid) —can be completed upon arrival
o Prepare to be fingerprinted
*Note: All coaches must provide verification of completion of the Care & Prevention of Athletic Injuries class, and paid coaches must also provide verification of CPR-AED certification that has not expired.

Tuesdays 8:30-10:30
Thursdays 8:30-10:30
Effective January 1, 2009, charges for fingerprinting will be as follows:
volunteer coaches - $10 fee for fingerprinting
paid coaches - $20 fee for fiingerprinting

FCPS coaches are encouraged to complete a
