Note: On the first day of school, students receive the Student Information Card which is pre-printed with previously supplied information for each student. Parents or guardians are asked to review, correct as necessary and return this card to the school immediately.
To comply with HIPAA privacy rules, student health & emergency information* is requested on a separate form that must be completed and returned annually.
